Payment Options
Retina Associates accepts many insurance plans. Please bring your insurance card with you at the time of your visit. If your plan requires a co-pay, this will be collected at the time of your visit. The billing department will process all insurance forms necessary.
Patients who are members of certain managed care plans, especially HMO's, may need to secure a referral/authorization from their primary care physician for services provided by Retina Associates of Cleveland, Inc. Failure to have the required referral/authorization may delay your visit with us and, in some cases, result in having to reschedule your appointment.
We accept checks as well as MasterCard, Visa, and Discover cards as accepted forms of payment of patient balances.
The doctors of Retina Associates of Cleveland are providers on most major insurance plans. If you need to know if your plan requires a referral or if the doctor you are going to see is listed as a provider on your particular plan, please call your insurance company before your visit to one of our offices.
You can be assured that Retina Associates will not charge a "facility fee" at any of our office locations.
